USB support must be enabled before config_distro_bootcmd.h is included

The built-in SMSC 95xx chip doesn't know its own MAC address. Instead,
we must query it from the VC firmware; it's probably encoded in fuses
on the BCM2835.

Convert the BCM2835 GPIO driver to use driver model, and switch over
Raspberry Pi to use this, since it is the only board.

Replace the custom $bootcmd with that from <config_distro_bootcmd.h>.
There should be no functional change, since the new generic $bootcmd was
derived strongly from tegra-common-post.h, after which this part of
rpi_b.h was modelled.
The #defines to enable/disable U-Boot commands/features were moved
earlier in rpi_b.h, so they are set up before config_distro_bootcmd.h
is included, since it tests whether certain features should be included
based on those defines.

Rework rpi_b's bootcmd (and sub-commands) to match Tegra's bootcmd as
much as possible. This will aid in a future patch which will create a
common header e.g. config_distro_bootcmd.h.
While at it, enable booting from extlinux.conf using the sysboot command.
The iteration and componentization currently makes a little more sense
for Tegra than RPi, but I'd still like to keep the two as similar as
possible. USB and networking support is coming to the RPi in due course,
and it'll all make more sense then anyway.

The Pi has no flash to store an environment in the usual fashion. However,
the user may wish to customize the environment. We know that the SD card
must be present, since that's where the boot ROM has loaded U-Boot from.
So, load uEnv.txt from there early during boot. This allows the user to
e.g. customize boot_targets, in order to automatically select network
boot.

Modify the rpi_b board to include the "distro defaults" header, so that
all the config options distros expect are enabled. Remove any #defines
that enable the same options from the rpi_b.h.

U-Boot names the Raspberry Pi board rpi_b. This means that the common
expression for DTB filename ${soc}-${board}.dtb expands to
bcm2835-rpi_b.dtb. However, the DTB generated by the Linux kernel is
bcm2835-rpi-b.dtb. Set $fdtfile in U-Boot's environment so that scripts
look for the correct DTB filename.
An alternative would be to rename the U-Boot board to rpi-b. However,
that change would be far more invasive, and end up affecting users (i.e
they'd have to change their U-Boot build commands).

Add a DT simple-framebuffer node to DT when booting the Linux kernel.
This will allow the kernel to inherit the framebuffer configuration from
U-Boot, and display a graphical boot console, and even run a full SW-
rendered X server.

Apparently, CONFIG_SYS_HZ must be 1000. Change this, and fix the timer
driver to conform to this.
Have the timer implementation export a custom API get_timer_us() for use
by the BCM2835 MMC API, which needs us resolution for a HW workaround.

Enable the SD controller driver for the Raspberry Pi. Enable a number
of useful MMC, partition, and filesystem-related commands. Set up the
environment to provide standard locations for loading a kernel, DTB,
etc. Provide a boot command that loads and executes boot.scr.uimg from
the SD card; this is written considering future extensibilty to USB
storage.

The firmware running on the bcm2835 SoC's VideoCore CPU manages the
display controller. Add a simple "LCD" driver that communicates with the
firmware using the property mailbox protocol. This configures the
display and frame-buffer to match whatever physical resolution the
firmware chosen when booting, which is typically the native resolution
of the attached display device, presumably unless otherwise specified
in config.txt on the boot media.
Enable this driver in the Raspberry Pi board configuration.

There appears to be no implementation of flush_dcache_range() for
ARM1176, so explicitly disable dcache support to avoid references to
that function from the LCD core in the next patch. This was presumably
not noticed before simply because no drivers for the rpi_b were
attempting DMA.

The Raspberry Pi model B uses the BCM2835 SoC, has 256MB of RAM,
contains an SMSC 9512 USB LAN/Hub chip, and various IO connectors.
